So when i start my 87 integra rs i hear like a winding/rattle noise coming from around the distributor area and the transmission what could that be?

Could be the vacuum advance weights in the distributor. Sometimes the springs on those guys like to break.

Have you checked to see if the rotor is secure? Members have mentioned the set screw being loose or missing. lol

What trans do you have in the car? Automatic or 5 speed?

Sounds like the torque converter is going out on you. The 86-87 auto trans has an internal lock-up solenoid and the 88-89 have an external electrically controlled lock up solenoid controlled by the ecu. So what I'm saying is you really can't swap one for the other without causing control problems. I've put a external solenoid trans in an 87' once before but it required a dash switch, relay and wiring to the solenoid. In town, I'd leave it off. On the freeway I'd lock it up but, whenever you come off the off ramp, you'll need to switch it off again otherwise you might have the engine stall when coming to a stop.
